Underwear that grant you use of the spells : Power Word Kill, Power Word Kill (Long Rest), Godly Power Word Kill and Godly Power Word Multi-Kill respectively.

Tested and working on game version 4.1.1.3622274.

Power Word Kill Uses 1 Action Point, has a 60ft/18m range and is a guaranteed Kill on creatures with or under 100 Hit Points.

Power Word Kill (Long Rest) Uses 1 Action Point once per Long Rest, has a 60ft/18m range and is a guaranteed Kill on creatures with or under 100 Hit Points.

Godly Power Word Kill is free cast, has a 120ft/36m range, ignores line of sight and is a guaranteed Kill.

Godly Power Word Multi-Kill is free cast, has a 120ft/36m range, ignores line of sight and is guaranteed to Kill up to 5 creatures.

Underwear can be found in the Tutorial Chest on the Nautiloid or purchased from Trader Arron at the druids grove, Quartermaster Talli at Last Light Inn and Gloomy Fentonson in the Lower City for 2 gold each.

I use LaughingLeader's BG3ModManager from GitHub to "install" my mods

Mod Fixer is set as required but im not sure if it actually is as my game breaks when i removed the Mod Fixer.
